Moanalua Elementary will be providing all students with the opportunity to participate in the State of Hawaii, Department of Health and Department of Education Influenza Vaccine Program.  If you choose to register your child, he/she will be vaccinated with a “Flu Shot” or flu “Nasal Spray” on Monday, October 25, 2010.

The authorization forms were sent home with your child last week. If you choose to have your child participate, please complete and submit the appropriate paperwork by Thursday, September 9, 2010.
